Diabetes Education – Take Control Today!


Valley Health has Certified Diabetes Care and Education Specialists (CDCES) on staff to help you take control of your diabetes. CDCES are health professionals who have demonstrated their knowledge and skill in diabetes self-management education by providing at least 1,000 hours of patient education, passing a rigorous exam and maintaining this highly respected credential in the healthcare industry.

Successful diabetes self-management involves knowledge about a broad range of topics such as healthy eating, how to take medication as directed, fitting in exercise and coping with stress. It’s not easy to gain all of the necessary knowledge by yourself. With the help of a CDCES, you can learn how to effectively manage and improve your health by developing a plan. Ask for a referral from your provider today. This service is offered at a number of locations. Please talk to your local Valley Health site for availability.

Our CDCES are Megan Peterson, RPh, Clinical Pharmacist, and Penny Skidmore, RN, BSN.

Most insurances cover the cost of meeting with a CDCES. If you are not sure, contact your insurance company directly.

Chronic Care Management (CCM) – Program for Medicare Patients


Offered at Valley Health – HuntingtonWestmorelandTeays ValleyHartsHurricaneMiltonEast HuntingtonWayneUpper KanawhaFort Gay, and Stepptown CCM provides Medicare patients individualized care designed to improve health and reduce spending for individuals with two or more chronic health conditions.

Examples of Chronic Conditions: Alzheimer’s disease and related dementia; Arthritis (osteoarthritis and rheumatoid); Asthma; Atrial fibrillation; Autism spectrum disorders; Cancer; Chronic obstructive pulmonary disease (COPD); Depression; Diabetes; Heart failure; Hypertension; Ischemic heart disease; Osteoporosis

Our goal is to make sure you get the best care possible. We can help coordinate your visits with other doctors, facilities, lab, radiology or other testing; we can talk to you on the phone about your symptoms; we can help you with the management of your medications; and we will provide you with a comprehensive care plan. A nurse will be assigned to guide you through your care process. This same nurse will routinely contact you and be involved with all aspects of your health treatment.

Medicare covers most costs of CCM. Talk with your insurance company directly to find out what your co-payment will be.

Black Lung


Valley Health Systems provides specialized respiratory care and ancillary services for patients with occupational lung diseases. Three of Valley’s primary care centers, Upper KanawhaHarts and Stepptown, offer services to workers with black lung, silicosis and asbestosis.  Valley services include an array of diagnostic, treatment and benefits counseling options.

 

Harmony House – Healthcare for the Homeless


Valley Health has a partnership with the Cabell-Huntington Coalition for the Homeless to provide healthcare to those who are homeless in the Huntington area.

Harmony House is a multipurpose day shelter, providing case management, free primary healthcare, including eye and dental care, mental health and substance abuse counseling, outreach, life skills and employment programs, transportation and basic needs such as showers, personal care items, clothing, mail and telephone service, a place to rest and more.  The Coalition serves all homeless people in Cabell County and is recognized nationwide as a model program.

Valley Health – Harmony House has a certified family nurse practitioner on-site who provides healthcare including medications, lab tests and referrals when needed. Dental care is also offered to Harmony House clients at a nearby Valley site.
